Medical Records

Medical Records serves the community Monday through Friday from 8:30 am until 4:30 pm.

When requesting your Medical Records we ask that you give us a 24-hour notice. The charge for retrieving your Medical Record is a $5.00 search fee, $1.00 per page for the first 25 pages, $.50 a page thereafter and $7.00 a copy for radiology films. When picking up your records please bring a picture identification with you.

Medical Records is responsible for claims from Lawyer's Offices, Disability Determination, Other Doctor's Offices, Insurance Companies, etc.

We also code all patient, outpatient and emergency room visits.

We have a full-time Transcriptionist who is responsible for typing all the dictation from the Doctors.
